Community Wellness Information

The Community Wellness Award is presented to an individual, company, or organization that is a "pacesetter" in the area of community wellness in the Greater Baton Rouge community.  Community wellness is defined as the promotion of healthier lifestyles and includes corporate wellness programs and physical fitness programs.  The individual, company, or organization must be a resident or company from the Greater Baton Rouge area.

Community Wellness Award Nominations

Nominations for the 2009 Providence Corporate Cup Community Wellness Award will be accepted until April 10, 2009.  If you know of an individual or company that is a pacesetter for community wellness in the Great Baton Rouge business community, please submit a nominee!

Community Wellness Nominations Must Include

  1. Name of Individual/Company/Organization nominated
  2. Contact information of the nominee (name, phone number, and e-mail address if applicable)
  3. Explanation of accomplishments, goals, and details of wellness program.
  4. Name, phone number, and e-mail address of person submitting the nomination
